• DO NOT ASSUME THAT TEAMS ARE NOT TRAINING BECAUSE THE FIELD IS BLANK.  YOU MUST CONTACT THE PRESIDENT TO ENSURE THAT NO OTHER TEAM IS TRAINING BEFORE MOVING TO THAT TRAINING TIME.

Please carefully read here to find out what to do if you are injuried either at training or during a game. 

1. Download and complete the Player Injury Report. (This may be completed by your team manager on your behalf if injury is serious)
2. Fill in as many details as soon as possible after the event occuring.
3. You need to email this to the SUNNYBANK SAINTS CLUB SECRETARY - not the QCSA.  All correspondence goes via our club Secretary.
4. Advise the SUNNYBANK SAINTS CLUB SECRETARY if you require an INSURANCE FORM for any possible medical expenses and further claims.  We can provide you with these forms and the current policy/entitlements. (QCSA Website will also have details of policy).
5. Keep notes in a diary of your injury and any pain caused.
